» If you do have COVID symptoms, such as fever or shortness of breath, please do not enter our building and instead call (231) 737-0411 for more instructions.

If your child is a new paitnet, or it is your annual visit please arrive 15 minutes before your appointment to complete and submit paperwork. NICHQ Vanderbilt Assessment

The NICHQ Vanderbilt Assessment Scales are used by healthcare professionals to help diagnose Attention Deficit Hyperactivity Disorder (ADHD) in children between the ages of 6 and 12 years. It is an effective tool that has been at the forefront of supporting children and families affected by ADHD.

Download and print out a Parent Form for each parent. The parents should complete the Parent Form individually.

The Teacher forms can be downloaded and printed for the child’s Teacher(s) to fill out.

To do the assessment, we must have completed forms from each parent and at least one teacher.

Ages & Stages Questionnaires

The Ages & Stages Questionnaires are designed to help assess your child’s social and emotional development. It reliably identifies strengths and developmental milestones as well as areas for attention, relying on parents as experts. It’s easy to use and family-friendly.
